Slate Belt Health & Rehabilitation Center, owned by SHG Rez, LLC as of the first quarter of 2026, is situated in Northampton County, according to data from the Centers for Medicare and Medicaid Services (CMS). SHG Rez, LLC has held ownership since July 1, 2018.

The facility operates as a for-profit nursing home.

During this period, the center held an overall CMS rating of four, exceeding Pennsylvania’s statewide nursing home average of 3.0. CMS ratings are based on key factors such as results from health inspections, staffing numbers, and the quality of resident care.

Data shows Slate Belt Health & Rehabilitation Center averaged care for 108.4 residents per day in the first quarter of 2026 and offered 119 beds. The center currently is not hospital-based.

Throughout the first quarter of 2026, the nursing home recorded no fines or penalties.

Northampton County had seven for-profit nursing homes in total.

U.S. Census Bureau estimates show the nation’s older adult population continues to expand as numbers of younger age groups fall. From 2020 to 2024, Americans age 65 and older grew by 13%, while those under 18 dropped by 1.7%. The country’s older adult segment has grown substantially over the last century, rising from 4.9 million in 1920 to more than 61 million by 2024.

A study from 2021 indicates that almost half of those age 65 and older—about 28 million people—could need long-term care services. The research also forecasts that more than a third may require nursing home care in the future.

“Living a long life is something that many of us want and could get,” said Jesse Slome, executive director of the American Association for Long-Term Care Insurance, an insurance education group. “But when we live a long life, the chances of us needing long-term care increase exponentially. But when you need that type of care, there are limited options.”
